If you need help with daily living, waiver services may be right for you. Waiver services help you meet your daily needs. Waiver services help you be as independent as you can be in your own home. You may qualify if you are:
- 18 years or older
- Fully eligible for both Medicare and Medicaid
- Enrolled in a MyCare Ohio plan
- In need of intermediate or skilled care, according to State of Ohio rules
Waiver services include a variety of things. They include home delivered meals, nutrition counseling, home modification and more.
With waiver services, you will have both a Care Manager and a Service Plan. Your Service Plan lists the services that have been approved for you. It shows the name of the agency or person who will provide each service. It also shows how often you get the service. You can print a copy of your Service Plan. Go to the Member Service Plan Log-In Page and follow the directions.

If you are a CareSource® MyCare Ohio (Medicare-Medicaid Plan) member with waiver services, you may get help from many different people. These people may give you personal care or help with daily chores in your home. They help you stay healthy and safe.
If the care you get ever causes you harm or makes you afraid, this is called an “incident.” We want you to know how to report incidents. Your health and safety is very important to us. To learn more about how to report incidents, please see your Waiver member handbook on our Plan Documents page.
CareSource® MyCare Ohio (Medicare-Medicaid Plan) is a health plan that contracts with both Medicare and Ohio Medicaid to provide benefits of both programs to enrollees.
