Disease Management Programs
Members identified as high risk will have a nurse assigned to their case to help educate, coordinate and provide resources and tools to help the member reach their health care goals.
Our free Disease Management programs help our members find a path to better health through information, resources and support.
We help our members through:
- The MyHealth online program for members 21+ to participate in a journey to improve their health
- Access to health education and an Interactive Health Library within the MyLife app which has helpful tips and information
- Coordination with outreach teams
- One-to-one care coordination (if they qualify)
Members with specific disease conditions are identified by criteria or triggers such as emergency room visits, hospital admissions, and the health assessment. All ages (children, teens, and adults) are eligible. These members receive notifications via text, email, or letter when new educational materials are available in their MyLife App to view, prompting them to sign in and review the digital content. The materials are available in English and Spanish. Any member may self-refer or be referred into the Disease Management Programs to receive condition-specific information or outreach.
Benefits to Members and Health Partners
Members identified in the Disease Management programs receive help finding the appropriate level of care for their condition, and they are encouraged to actively participate in the patient-provider relationship. The programs improve the percentage of CareSource members who receive their recommended screenings.
