Up-to-date contact information is critical to process your claims. In addition, it ensures our directories are up-to-date and reduces unnecessary calls to your practice. 

Update Your Information

Complete the Health Partner Change Request Form to alert CareSource to a change within your practice, for example:

  • Adding or deleting a provider to a group
  • Changing an address or phone number
  • Adding new restrictions or capacity limitations

You can email, fax or return the form to a CareSource Provider Representative.

CareSource is an HMO with a Medicare contract. Enrollment in CareSource Advantage Zero Premium™ (HMO), CareSource Advantage® (HMO), and CareSource Advantage Plus™ (HMO) depends on contract renewal.

NOTE – To change your Tax ID number, or update your IRS name, you must make those changes through an amendment to your contract, not through maintenance. You can make those changes using the New Health Partner Contract Form.
