Care Coordination
We work with you to help you achieve what matters most. All members get a Care Coordinator. They help you:
- Schedule provider visits
- Find providers
- Understand your benefits
- Get transportation to health visits
- Find community resources
- And more!
Your Care Coordinator will work with you to develop a person-centered support plan and partner with you in your health care journey to ensure we support your physical, mental and social needs. We use a team approach with you at the center to support your needs. Your care team is a group of people who can help you meet your goals for a healthy life. Your care team may include providers, specialists, loved ones, neighbors and community resources.
Your Care Coordinator is an advocate for your goals and help you navigate the health care system. HAP CareSource MI Coordinated Health Care Coordination coordinates your Medicare and Medicaid benefits so you can have peace of mind and focus on your well-being and healthy life.
Call Member Services at 1-833-230-2057 (TTY: 1-833-711-4711 or 711) if you don’t know who your Care Coordinator is. You can also see who your Care Coordinator is when you log into your CareSource MyLife account. First, click the “Get Help” tab. From there, select “Care Team.”
Disease Management
If you are living with chronic health issues like diabetes, asthma, high blood pressure, or high cholesterol, our disease management programs are just for you.
Our disease management programs can help you learn more about your health. They can also help you manage your health condition. You can choose to be part of a program or we may hear from your doctor, pharmacy or other provider that you would benefit from this program. We want to help you be healthy and well.
