Get more of what you need with the Healthy Benefits+ card, which lets you easily access and use your allowances and rewards.
Healthy Benefits+
With your Healthy Benefits+ card you have $300 per month to purchase qualifying items, services, and accessories at eligible locations. Your allowance rolls over each month, and funds don’t expire until the end of 2027. You can spend it on:
- Over-the-counter items
- Dental
- Vision
- Hearing
For those approved for Special Supplemental Benefits for the Chronically Ill (SSBCI), you may use the Healthy Benefits+ card for the following qualifying items and services at eligible locations:
- Healthy Food*
- Utilities*
- Rent & Mortgage Assistance*
- Household Cleaning Supplies*
- Personal Care Items*
- Haircut Barber Services*
Validate your condition by completing your Health Risk Assessment (HRA) with your Care Coordinator or have your provider complete and submit a provider attestation form.
