Get support tailored to your unique needs. We are a single point of contact working with you and your providers to coordinate your care. All members get a Care Coordinator. They can help you:

  • Schedule provider visits
  • Find providers
  • Understand your benefits
  • Get transportation to health visits
  • Fill out the Health Risk Assessment (HRA)
  • Find community resources
  • And more!

Disease Management

We offer a disease management program to help you learn about your health and managing your specific health conditions. We have materials with helpful tips and information to manage your condition and promote self-management skills.

All ages are eligible to participate in the program. Any member may self-refer or be referred into a disease management program to receive condition-specific information or outreach. We may receive information from your doctor, pharmacy or other health care provider letting us know you would benefit from a program.
