My CareSource Rewards

Earn rewards for doing healthy activities with My CareSource Rewards®. You must have both your Medicare and Medicaid with CareSource to be eligible for this benefit.
Want to get both your Medicare and Medicaid with CareSource? Call 1–855-475-3163 (TTY: 1-833-711-4711 or 711). We are open Monday through Friday, 8 a.m. to 8 p.m. Eastern Time (ET), and from October 1 to March 31, the same hours, seven days a week.
How does it work?
- Get your Healthy Benefits+ card.
If you are a CareSource MyCare Ohio member with both Medicare and Medicaid benefits, you are automatically enrolled. You will get your Healthy Benefits+ card in the mail when you join the plan. This is the same card you use for your monthly Healthy Benefits+ funds.
- Complete healthy activities.
Rewards are added to your card when you complete healthy activities. It can take up to 60 days from when we get the claim for the reward to show on your card. See the list below to learn how to earn rewards.
- Redeem your rewards.
Go to HealthyBenefitsPlus.com/MyCare-SNP to see where to shop, check your balance, and learn more.
You can also log in to your CareSource MyLife account to track your rewards.
What Rewards Can I Earn?
Rewards are earned once each year unless noted.
Rewards for Immunizations
| $50 | Flu shot. Earn $50 for each shot up to two times each year. |
Rewards for Preventive Care
| $50 | Annual physical exam. |
| $10 | Annual dental exam. |
| $10 | Sign up and link your plan in CareSource MyLife. |
| $50 | Colorectal cancer screening. |
| $50 | A1C test. Earn $50 for each test up to two times each year. Must have a diabetes diagnosis. |
| $25 | Kidney health evaluation. Must have a diabetes diagnosis. |
| $25 | Retinal eye exam with an eye care provider. Must have a diabetes diagnosis. |
| $75 | Breast cancer screening (mammogram). For women ages 40 and older. |
| $50 | Follow up visit for Osteoporosis after a diagnosed fracture. For women ages 67 and older. |
| $50 | Engagement with your Care Coordinator after a hospital discharge. You must engage within 30 days of the discharge. |
| $50 | Complete your Health Risk Assessment (HRA). |
*Rewards are subject to change. Rewards may vary by age, gender, and health needs. Rewards may expire based on date of issuance. If you are no longer a CareSource member, your access to the rewards portal will be deactivated, but the reward amount will remain on the card.
