CareSource offers MyHealth as a benefit to members ages 18 and older. Through MyHealth, you have access to:

  • Interactive health assessments.
  • Personalized health tools, guides and videos.
  • Online tools to set and track health and wellness goals.
  • Nutrition and exercise.
  • Preventive health.
  • Emotional health.
  • Tobacco and drug cessation and limiting alcohol intake.

You won’t have to wonder where to begin or what to do next. MyHealth is your guide. Come back to the tool often to track your progress.

MyHealth Rewards

Take an Active Role to Better Your Health!

Adults ages 18 and older may be able to earn rewards through the MyHealth Rewards program. Earn rewards when you get certain preventive care screenings. Redeem your rewards to get electronic gift cards to well-known stores such as Panera, TJ Maxx®, Wal-Mart® and many more.

  1. Log In.
    Earn, track and view your rewards balance in MyHealth through CareSource MyLife.

    Rewards are subject to change. The rewards available to you will vary depending on your health care needs. Note: not all reward activities are covered services annually. You may be responsible for the cost if you do not check with CareSource or your primary care provider (PCP) before receiving services.

  3. Redeeming Rewards is Easy
    • Complete one or more reward activities.
    • Your provider’s office will submit a claim for the activity to CareSource
    • CareSource will process the claim and add rewards points to your MyHealth Rewards account. Please allow 60 days for processing.
    • Log into your CareSource MyLife account to redeem your points. Get electronic gift cards to stores such as Old Navy®, TJ Maxx®, Walmart®and more!

Notes:

  • Rewards earned in the current year must be redeemed before mid-December of the following year. If not redeemed by the deadline, the rewards will expire.
  • If you stop being a CareSource member, your access to the rewards portal will be deactivated and your rewards will no longer be available.
