Care & Disease Management
CareSource offers programs to help you learn about your health and how you can better manage your specific health conditions. Our goal is to make sure you have the right tools to stay as healthy as possible. These programs are available to you at no cost. Participation is voluntary, you can opt-out of the program at any time.
Care Management
CareSource has nurses and social workers who can work with you one-on-one to help coordinate your health and non-health care needs.
They can contact you by phone if:
- Your doctor requests one to contact you.
- You request a phone call.
- Our Care Management staff think their services would be helpful to you or your family.
Our staff is trained to help you and your family with any special medical problems. We help manage asthma, cancer, diabetes, and many other medical conditions. We can also work with you if you need help figuring out when to get medical care from your doctor, an urgent care center or the emergency room.

Disease Management
Our free disease management programs can help you learn more about your health and better manage your specific health conditions. We want you to have the right tools to stay as healthy as possible. As a CareSource member, you have access to:
- MyHealth® an online tool for members age 18 and older. Learn how to live a healthy life using the tools in this app. Take a health assessment, make a wellness plan and track your progress!
- Educational materials with helpful tips and information to manage your condition and learn self-management skills.
- Care coordination services.
- One-to-one care management.
All ages (children, teens, and adults) are eligible to participate. You can self-refer or be referred into a program. We may receive information from your doctor, pharmacy or other health care source to let us know you might benefit from one of our programs. We will send you materials related to your health condition along with tips and resources available to you.
