Get Contracted

Getting contracted to sell CareSource health plans in Wisconsin, is simple! Just follow these steps:

Send the following information to our CareSource WI Broker Support team at CGHC_CGHCSales@caresource.com or call us at 888-870-4717:

  • Your full name
  • Email
  • NPN
  • Agency up line
  • Agency NPN or Tax ID

For all other states: If you are interested in getting contracted in other CareSource states, you must be affiliated with a Field Marketing Organization (FMO). Once you’re appointed with an FMO, simply select the state you wish to contract with and follow the provided instructions.
