CareSource is a nonprofit health plan headquartered in Dayton, Ohio. As one of the largest Medicaid managed health plans in the country, CareSource serves over one million consumers. Ohio Medicaid

Ohio Medicaid consumers include families, children, pregnant women, adults age 65 and older and Ohioans who are blind or disabled at any age. If you are eligible for Ohio Medicaid, you and/or members of your family may receive health care benefits through CareSource. CareSource Just4Me

CareSource MyCare Ohio (Medicare-Medicaid Plan)

Ohio Home Care Waiver

The Ohio Department of Medicaid (ODM) has contracted with CareSource to manage the Ohio Home Care Waiver Program in northeast, southeast and central Ohio. The Ohio Home Care Waiver Program provides a cost-effective home and community-based service so that you can stay in your home and community.
