Care Coordination

HAP CareSource MI Health Link Care Coordination works with you to help you achieve what matters most. Your Care Coordinator will work with you to develop a person-centered support plan and partner with you in your health care journey to ensure we support your physical, mental and social needs.

Disease Management

HAP CareSource MI Health Link understands that living with a health condition can be hard. We are here to help you and better support your health. Our goal is to make sure you have the right tools to stay as healthy as possible.

We help our members through:

• Care Coordination with outreach teams.
• One-to-one Care Coordination (if they qualify).

We may receive information from your doctor, pharmacy or other health care source letting us know that you would benefit from a program. We will send you materials related to your health condition along with tips and resources to help you self-manage your condition.

Any member may self-refer or be referred into a disease management program to receive condition-specific information or outreach. Please call us if you would like more information about your health condition or would like to be part of a disease management program. Care Coordination

Your health and wellness are important to us at HAP CareSource MI Health Link. One of the advantages of HAP CareSource MI Health Link is that every member is assigned a Care Coordinator. We provide person-centered care coordination. We help you develop a personalized plan for your health. We use a team approach with you at the center to support your needs. Your Care team is a group of people who can help you meet your goals for a healthy life. Your Care team may include providers, specialists, loved ones, neighbors and community resources.

It’s important to have someone you can trust to help you stay healthy. Your Care Coordinator will help coordinate all your health care needs and assist with communications between the Care team members. They will help you take the best care of yourself and understand the importance of your medication plan, lab work or a medical test. Your Care Coordinator will be an advocate for your goals and help you navigate the health care system. HAP CareSource MI Health Link Care Coordination will coordinate your Medicare and Medicaid benefits so you can have peace of mind and focus on your well-being and healthy life.

Whether it’s finding a community resource, making health appointments or assessing next steps for a health goal, your Care Coordinator will lead the way. Together with your Care team, they can oversee your needs and coordinate your care.
