What is Advance Care Planning?
Advance care planning is the process of preparing for decisions about your health care if you become seriously ill. It involves talking with your providers, family and other important people in your life. You can also put these decisions in to writing by completing an advance directive. This is a legal document that allows your care wishes to be followed even if you cannot tell others what you want. You can make changes to your advance directive(s) at any time.

You may want legal advice to learn more about documenting your wishes through an advance directive. Health care laws do differ by state. These legal services may not cost you anything based on certain requirements. Keep any documents in a safe place where your family can get to them.
