A Member Services representative will be happy to answer your questions Monday through Friday, 7 a.m. to 7 p.m. unless otherwise stated below. Kentucky Subrogation Requests
All inquiries regarding subrogation lien amounts or member record requests must be accompanied by a HIPAA compliant authorization for the release of member information and must be mailed to Humana – CareSource, Attn: Legal Department, 230 North Main Street, Dayton, OH 45402, or inquiries may be faxed to: 1-800-605-0137.
Hours of Operation and Holidays
CareSource is open for business 8 a.m. to 5 p.m. Monday through Friday, except for holidays.

- Thanksgiving Day and the day after
- Christmas Eve and Christmas Day
If the holiday falls on a Saturday, the company will be closed on the Friday before. If the holiday is on a Sunday, we will celebrate the Monday after.
